Personal care support

Blood pressure (BP) monitoring
Ongoing observation to maintain healthy blood pressure levels and prevent complications.
Medication administration
Professional assistance in administering prescribed medications safely and on time.
Pressure area care
Preventative skin care and repositioning to reduce the risk of pressure injuries.
Continence management
Dignified support for bladder and bowel care, ensuring comfort and confidence.
Oxygen therapy
Safe administration and monitoring of oxygen to assist with respiratory conditions.
End-of-life care
Compassionate, holistic support focused on comfort, dignity, and emotional wellbeing.
Behaviour management
Gentle, evidence-based strategies to support clients experiencing behavioural changes.
Medication management
Comprehensive oversight to ensure medication safety, compliance, and effectiveness.
Morphine injections
Professional administration of morphine for pain management under clinical guidance.
Post-operative care
Support and monitoring after surgery to promote safe recovery and healing at home.
In-home respite care

Mobility assistance

Assessments by Registered Nurses (RN)

Referral Pathways
Referrals are accepted only from:
- Your General Practitioner (GP)
- Treating doctor in hospital
- Hospital discharge planner
- Nurse practitioner specialising in community nursing
Referral Validity
- Hospital referrals: valid for 6 weeks
- Referrals from GP, nurse practitioner, or hospital: valid for 12 months
Referral Process
- Get a referral from your GP or hospital
- Email referral to comnursing@threestrandcare.com.au.
- RN will arrange a visit
- RN will visit to assess and allocate service
- Services will be commence.
We welcome DVA clients, DVA Health Cards (Gold and White) are accepted as payment upon a GP referral. DVA Health Cards (Gold and White) are accepted as payment upon a GP referral .
