Position Overview:
As a Disability Support Worker, you will play a crucial role in implementing training and support services that enhance the living, employment, social, and recreational skills of individuals with disabilities.

Key Responsibilities:
- Facilitate Participation: Support participants in engaging in age-appropriate activities, including employment, in-home support, and community access.
- Direct Care Assistance: Provide individualized care as outlined in service plans, ensuring each participant’s needs are met with dignity and respect.
- Safety Monitoring: Regularly assess the safety and well-being of participants, promptly reporting any concerns to management.
- Training & Support: Deliver one-on-one or small group training tailored to the specific needs of each participant.
- Documentation: Accurately record relevant information regarding the support provided in participant notes, ensuring compliance with all documentation standards.
- Lifestyle Service Planning: Collaborate in the Lifestyle Service Planning process, monitoring outcomes to ensure participants achieve their goals.
- Team Collaboration: Work closely with participants, their families, case managers, and other professionals to support individualized goals.
- Program Development: Assist in the development, implementation, and evaluation of training and behavior programs to help participants reach identified objectives.
- Key Worker Role: Take on the responsibilities of a key worker for designated participants, preparing monthly progress reports.
- Communication & Teamwork: Engage actively in a team environment through open communication, participation in staff meetings, and policy reviews.
- Performance Development: Participate in Performance Development meetings to identify and pursue ongoing training and support opportunities.
- Shift Notification: Inform the service of any inability to work scheduled shifts as early as possible.
- Case Reviews: Contribute to case reviews alongside Employment Consultants and Facilitators.
